DUTIES &
Responsibilities:
oAssist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards oDevelop and manage the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures oAssist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient/resident oDevelop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents oKeep abreast of current federal and state regulations as well as professional standardsQUALIFICATIONS & REQUIREMENTS:
oMust have a current Social Work license, in good standing with the state (as required) oBachelor's Degree in Social Work strongly preferred oMust have at least 3
years of experience in a long term care facility oMust be able to organize and prioritize many tasks effectively oIdeal candidate must be a skilled communicator, director and motivator
