The HCHC RCC is a 40 bed inpatient program that offers primary care, case management, wound care, and other supportive services to clients who are experiencing homelessness made possible through partnership with the City of Las Vegas.

Summary:
The Community Health Worker (CHW) will work to educate, empower, and improve outcomes among Hope Christian Health Center patients affected by hypertension and/or diabetes.
The CHW will work as an outreach coordinator for colorectal cancer screening.
The Community Health Worker, a grant funded position, outreaches to these high-risk patients in the clinic, engages with patients and feeds information back to the Management team to develop a care plan, links patient with needed resources in the community and services within the clinic.
You must be vaccinated against COVID-19 to be considered for this position Medical Assistant educational certificate or national MA certification is required.
Must obtain Community Health Worker certificate with 180 days of being hired.
DUTIES AND
Responsibilities:
Administrative Duties Promotes and demonstrates Hope values of:
Gospel motivation, Prayer, Excellence, Stewardship, Patience, Thankfulness, Humility, Service, Dignity through Relationship and Hospitality and Joy The CHW will track and improve colorectal cancer screenings to ensure > 80% of screened eligible patients are tracked from reminder to final diagnosis if applicable.
Maintain, and update as needed, EHR system provider reminder functionality to track Work with providers to ensure provider recommendation during clinic visit with 85% of due/overdue patients and track screening results.
Provide scheduling assistance and navigation to >95% of patients.
Conduct at least one (1) monthly program data review to identify due and overdue patients for colorectal cancer screening or diabetes screening.
Complete four (4) quarterly and one (1) annual (5 total) reports to track patients who receive a provider reminder for HTN, diabetes, or colorectal cancer screening measures.
CHW, track 100% of patients who are non-adherent with one (1) follow-up call or email (patients who need further follow-up to care have negative, incomplete, or inadequate results).
Finds and utilizes community resources for eligible patients.
Works with HTN case manager and clinical staff to communicate status of patients screenings/ results Protects HIPAA and maintains patient confidentiality.
Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complies with legal regulations Updates job knowledge by participating in educational opportunities; reading professional publications Attends and actively participates in staff meetings and participates in committee meetings as requested Maintains respectful, courteous, and appropriate communication and interpersonal skills with all staff members, volunteers, and patients Supports patient care activities by managing time effectively, utilizing resources appropriately, and exemplifying excellence in attention to detail Performs all other duties as assigned
Qualifications:
High School Diploma or equivalent-required.
Fluency in English and Spanish-required Medical Assistant Educational Certificate-required Community Health Worker Certificate-preferred Knowledge of Microsoft Office and experience with Excel spreadsheets-required.
