6 Quick Apply Full-time 8 hours ago Full Job Description About Hope Christian Health Center Hope Christian Health Center is a non-profit, faith-based organization operating two clinics in North Las Vegas, a mobile outreach team, and the HCHC Recuperative Care Center (HCHC RCC) in Las Vegas.
The HCHC RCC is a 38 bed inpatient program that offers primary care, case management, wound care, and other supportive services to clients who are homeless made possible through partnership with the City of Las Vegas.
Our mission is to glorify God as we demonstrate Christ's love to the Las Vegas community by providing excellent, affordable care to everyone regardless of their ability to pay.

Summary:
The Care Coordinator serves to help patients by processing referrals to specialty providers through the patients insurance coverage or by locating cost effective options for uninsured patients.
The Care Coordinator assists patients with overcoming barriers in obtaining specialty care-cost, transportation, scheduling an appointment, etc.
They follow-up with the patient to ensure the referral loop has been closed-the patient has been seen by the specialist and medical consultation reports have been received.

Responsibilities:
Administrative Duties Promotes and demonstrates Hope values of:

Verifies patient identity, medical insurance status, establishes if patient is eligible for Sliding Fee Scale while processing referral Provide referrals for continuing care and locate community resources for patients Always maintains and ensures patient confidentiality.
Provides accurate and detailed phone messages to providers and staff as needed.
Obtains patient consent/signature(s) when necessary.
Informs patient of steps needed for referral to be covered under insurance Relays detailed specialty referral information to patient Schedules patient appointments with specialist providers as needed Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complies with legal regulations.
Ensures front office, sliding fee schedule, and clinic policies and procedures (as applicable) are always followed.
Updates job knowledge by participating in educational opportunities; reading professional publications.
Attends and actively participates in staff meetings.
Supports patient care activities by managing time effectively, utilizing resources appropriately, and exemplifying excellence in attention to detail Performs all other duties as assigned.
Qualifications:
High School Diploma or equivalent:
required Medical Assistant Certification:
required Fluency in English and Spanish:
required Thorough understanding of health insurance (HMO, PPO, Medicaid, Medicare):
